ActiveConfirmed

Madagascar seasonal plague activity, 2026

Ongoing seasonal plague activity under review. Publicly verifiable reports now support at least 7 confirmed cases, including 3 confirmed pneumonic cases in Midongy Atsimo. Three deaths were previously reported in that cluster, but the latest account does not explicitly link the confirmed cases to the deaths, so this record does not publish a confirmed-death total.

Current assessment

Public reports citing Madagascar's Ministry of Public Health describe 4 confirmed plague cases since late July. A ministry-attributed update on 18 October said 3 of the 5 cases previously reported in Midongy Atsimo had been confirmed as pneumonic plague, no new case had been detected there since 12 October, and remaining suspected cases were awaiting laboratory results.

What is established

Multiple reports citing the ministry describe 4 confirmed cases in Ankazobe, Anjozorobe and Ambohimahasoa; the patients were receiving treatment and no deaths were reported among them.

About 200 contacts received preventive antibiotics and follow-up while active case finding, flea control and rodent-control measures were implemented.

Orange Actu relayed a health ministry update saying that 3 of the initially reported cases in Midongy Atsimo were confirmed as pneumonic plague and no new case had been detected there since 12 October.

Not yet confirmed

Two of the 5 initially reported Midongy Atsimo cases remain unconfirmed; the report says laboratory results for other suspected cases are still pending.

A local health official previously reported 3 deaths in one family. The latest account does not explicitly identify those deaths as the 3 confirmed pneumonic cases, so the death classification still needs an official case-level clarification.

Still unknown

The clinical forms of the 4 earlier confirmed cases, final results for the remaining suspected cases in Midongy Atsimo, classification of the 3 reported deaths, and status of separately reported suspected bubonic cases in Iakora remain unclear. The Iakora reports are not included in this record's counts.
